Discontinue a Plan

Why are we asking for this information? So we can contact you if there are any problems discontinuing your plan.
If you were issued an RFID tag then please enter the numbers on the back of the tag. The tag can be found on the bottom left corner of the front windshield. If you were not issued an RFID tag then you should enter your license plate with a hyphen then the state code afterwards. Check top of page for examples.
